PART II - BACKGROUND INFORMATION

  Purpose of Counseling


    The Intent of this counseling is to communicate a clear understanding to soldiers that they are to Drill with the UIC to which they are currently assigned.

PART III - SUMMARY OF COUNSELING
Complete this section during or immediately subsequent to counseling.


SSG M________, you currently drill at the UIC WAB1B9, ______ Det, as the acting PSG for a Chemical Platoon.

You have been drilling for approx. 2+ years with this Detachment knowing that you are:

a) Not currently 74D qualified.
b) Not currently assigned to WAB1B9
c) have been verbally directed to Drill with WAB1D3, where you are assigned.

You are hearby directed to begin drilling with your assigned UIC, in ___________, NLT JAN BTA.

Failure to attend BTA's at assigned UIC location will result in unsatisfactory participation (U's) status.


You are advised that continued misconduct or unsatisfactory duty performance of this nature will not be tolerated by the commander and may result in disciplinary action under the UCMJ and/or adverse administrative action to include separation IAW AR 635-200, prior to your normal expiration of term of service. You may also be issured an Honorable, General (Under Honorable Conditions), Uncharacteristic, or an Under Other Honorable Discharge Certificate. If you receive a General Uncharacterized, or an Under Other Than Honorable Discharge Certificate, you may encounter substantial prejudice in civilian life, and loss or delay in your eligibility for educational and/or other veteran's benefits. This counseling was conducted IAW AR 635-200, paragraph 1-18b, and AR 350-21, paragraph 5.

OTHER INSTRUCTIONS
This form will be destroyed upon: reassignment (other than rehabilitative transfers), separation at ETS, or upon retirement. For separation requirements and notification of loss of benefits/consequences see local directives and AR 635-200.
