Lost ID Card Counseling

PURPOSE: Event-Oriented Counseling: Loss of Military Property of the United States (Violation of Article 108, UCMJ); Violation of Army Regulation 600-8-14, Identification Cards for Members of the Uniformed Services.


KEY POINTS OF DISCUSSION: PFC___________, on or about 12 January 2015, you lost/misplaced your government issued identification card. This counseling is to inform you that you have violated the UCMJ by losing your identification card and that your performance needs to improve.

You may refer to AR 600-8-14 for penalties for misuse of ID cards: 1.7. Penalties for Misuse of ID Cards. Any person willfully altering, damaging, lending, counterfeiting or using ID cards in an unauthorized manner is subject to fine, imprisonment, or both according to Title 18, U.S.C., Sections 499, 506, 509, 701, or 1001.

You may refer to the Manual for Courts-Martial (MCM), Article 108: e.(2) Through neglect damaging, destroying, or losing, or through neglect suffering to be lost, damaged, destroyed, sold, or wrongfully disposed of, military property. (a) Of a value or damage of $500.00 or less. Confinement for 6 months, and forfeiture of two-thirds pay per month for 6 months.

Losing your ID Card is a serious incident and can cause serious harm in the wrong hands. At this time, I am not recommending any actions be brought against you.


PLAN OF ACTION: __________Remember the ID card holder responsibilities IAW AR 600-8-14 (Para 1.19).

1.19.1. Carry their ID card at all times.
1.19.2. Surrender the card when a military authority requires it for identification or investigation or when in confinement.
1.19.3. Attempt to retain ID card, if captured as a prisoner of war (military only).
1.19.4. Notify the ID card issuing activity when there is any change in their status that could change their ID card benefits.
