Monthly Counseling, Driver

DUTY PERFORMANCE: Your duty performance for the month of April is improving. You are knowledgeable when it comes to your duties. Continue to ask questions and seek more information regarding your role. Take the initiative to find success in your career.

PHYSICAL FITNESS: You are in great physical shape. Your PT standards are higher than most. Keep doing PT on your own and strive to improve your score on the APFT.

ACCOUNTABILITY AND RESPONSIBILITY: Always be at the right place at the right time. You will always be accounted for so this has not been an issue.

LEADERSHIP: You execute well over the standard as a driver. You have the mindset of a leader and perform your duties to the next position.

DISCIPLINE: You have a positive impact on the discipline of the unit. You do not have any misconduct against you. You are always doing the right thing. Your room is always straight and your uniform is the standard.

PERFORMANCE SUSTAINS: Continue to lead the other drivers and perform above your position.

SOLDIERS GOALS: Post your short term and long term goals in the "plan of action" section of this counseling statement.

PERSONAL ISSUES: If you do have any personal issues, do not hesitate to contact me. If not, contact the other chiefs or the PSG. Always follow your chain of command.
