MEMORANDUM FOR STATEMENT
1. I respectfully submit this statement for consideration by the Physical Evaluation Board. I earned my commission through dedication, discipline, and a strong commitment to serving the United States Army. Throughout my training and service, I have consistently demonstrated the values and professionalism expected of an Army Officer.
2. While I acknowledge the medical challenges I am currently managing, I firmly believe that with appropriate time, treatment, and continued support, I can return to performing my duties to the standard required of my position. This includes successfully completing the Army Fitness Test, fulfilling routine officer responsibilities, and meeting the physical and cognitive demands associated with strenuous tasks. I also believe that, with proper recovery and stabilization, I can safely and effectively perform my duties in both garrison and deployed environments.
3. My goal is to continue serving honorably and to contribute meaningfully to my unit and the Army as a whole. I remain committed to improving my health, maintaining readiness, and upholding the responsibilities entrusted to me as a commissioned officer.
