Radiology Technician Monthly Counseling

DUTY PERFORMANCE: SPC Snuffy, your duty performance for the month of February needs improvement. Although you are proficient as a Radiology Technician, you have trouble completing assigned tasks. You are very knowledgeable when it comes to your duties but you tend to procrastinate or take the easy way out when you do not know something. I encourage you to ask others that are working toward promotion about what to study. I am also available if you have trouble understanding the regulations or need someone to quiz you but I encourage you to study on your own. Remember that initiative and self improvement have a big impact on your success in the Army. Make sure you report to work well rested, alert, and able to work. You will not consume alcohol within 12 hours of reporting for duty.

PHYSICAL FITNESS: Since we do not have a structured physical fitness schedule, keep in mind that you should be able to pass a PT test at any given time. Make sure that you are conducting PT on your own time.

ACCOUNTABILITY AND RESPONSIBILITY: I have not had an issue with you reporting to work on time. Continue to do a good job and remember to always be at the right place at the right time in the right uniform. If you are ever running late, notify your chain of command as soon as possible.

PROMOTION: SPC Snuffy, until my assessment of your performance improves, I will not recommend you for promotion. Although you have demonstrated that you are not ready for promotion at this time, I encourage you to start studying and completing correspondence courses in your spare time.

LEADERSHIP: I worked with you when you were assigned as Shift Leader. You took your job seriously and executed that position efficiently. Great Job! If you see a buddy in trouble, help them get the help they need.

DISCIPLINE: You have been walking a thin line. You have an issue with divulging unnecessary information and are on the brink of disrespect, even if you consider it to be "joking." You need to improve your situational awareness and consider if something might be interpreted as unprofessional before you say it. I want to see you succeed but you cannot continue to have such a careless attitude when it comes to your comments. You will conduct yourself in a professional manner ON & OFF DUTY. You will render the proper respect to others, especially those senior in rank.

PERFORMANCE SUSTAINS: Continue to be where you need to be at the right time.

SOLDIERS GOALS: I would like to revisit your long and short term goals to see where you're making progress, and what areas needs improvement.

TRAINING: Stay on top of training and MEDPROS. Review your records regularly and re-certify or make an appointment before expiration.

ROOM INSPECTIONS: Your room must be kept clean and orderly and in a high state of readiness at all times. You should be prepared for an unannounced inspection at all times.

PERSONAL ISSUES: Don't hesitate to come to me with any issues you may have. My door is always open and I will do my best to help you in any way I can. If I can't help you, I will point you in the right direction and ensure you get the help you need.
