- Name and Title of Counselor: [Your Name/Rank]

PART II - Background Information

- Purpose of Counseling: Initial counseling for exceeding Army Body Composition Standards (AR 600-9) while on deployment.

PART III - Summary of Counseling

Key Points of Discussion:

On 21 August 2025, you were weighed and measured in accordance with AR 600-9, The Army Body Composition Program. Your height is 68 inches and your weight is 234 pounds. The maximum allowable weight for your height is 179 pounds. A body fat assessment was conducted using the tape method, resulting in 32% body fat. The maximum allowable body fat percentage for your age group (28-39 years old) is 24%. You exceed the standard by 8%.

You are expected to:
- Take immediate steps to improve physical fitness and body composition while deployed.
- Participate in remedial physical training sessions directed by leadership.
- Actively monitor your diet, hydration, and lifestyle choices.
- Submit to periodic weigh-ins/taping during this deployment.
- Work toward returning to Army standards (24% body fat or less) as soon as possible.

Failure to make progress during deployment will be documented. Upon redeployment, you will be re-screened, and if you remain over the standard, you may be formally flagged and enrolled in the ABCP. Continued failure can result in a Bar to Continued Service or separation under AR 635-200, Chapter 18.

Plan of Action:

- Soldier will attend all unit remedial PT sessions.
- Soldier will maintain a weekly log of personal workouts and diet adjustments.
- Soldier will submit to monthly weigh-ins/taping while deployed.
- Soldier will review Army nutrition/fitness resources provided by leadership.
- Counseling packet will be maintained and updated monthly to reflect progress.

Leader Responsibilities:

- Provide command-directed remedial PT sessions.
- Conduct and document weigh-ins monthly.
- Provide mentorship and monitor progress.
- Reassess Soldier's status upon redeployment and take further administrative action if necessary.

Session Closing:

You are being counseled for failing to meet Army body composition standards while on deployment. You are required to take corrective action now, and I will monitor your progress throughout deployment. Failure to meet standards can and will result in formal adverse action upon redeployment.
