Memorandum; Falsifying Security Records

The purpose of this Memorandum for Record is to document discrepancies observed in the on-call duties performed by SPC P_____ during the period 17-24 June 2026. On Wednesday, 17 June 2026, SPC P_____ took possession of the on-call book and was officially placed on call. He had originally been scheduled to begin on Monday, 15 June 2026, but due to health issues, his start date was delayed until 17 June. From 17 June through 22 June 2026, SPC P_____ completed all required on-call checks.

On Tuesday, 23 June 2026, at approximately 1701, I, SGT H______, observed that the DA Form 702 for that date did not contain the required morning check time. Assuming this was human error, I reviewed the remaining DA Form 702 entries and confirmed that all forms reflected the same issue, no morning check was conducted or recorded for 23 June 2026. I photographed the discrepancies and forwarded the evidence to his first line supervisor, SGT L____.

The On-Call SOP clearly states that required checks must be conducted at 0445 and 1630 daily. Due to the discrepancy, I returned to the warehouse at 1901 on 23 June 2026 to determine whether SPC P_____ had completed his on-call requirements. Upon inspection, I observed that SPC P_____ had falsified the time entries on the DA Form 702 by entering the morning check time retroactively. Additionally, he had entered the date and time for the following day, Wednesday, 24 June 2026, marking the morning check as 0447, despite that date not yet occurring. I photographed this evidence and again forwarded it to SGT L____.

Based on the evidence and prior observations, I have reason to suspect that SPC P_____ falsified required on-call documentation and that this might not have been the first occurrence of such behavior.
