Event Oriented: As a new parent you are required to have a Family Care Plan (FCP) on file to ensure that your dependent(s) are properly and adequately cared for when deployed, TDY, or otherwise not available due to military requirements IAW AR 600-20, para. 5-3.

PART III - SUMMARY OF COUNSELING

KEY POINTS OF DISCUSSION:

As a new parent, you have new and additional responsibilities. You are responsible for the care of your dependent(s) under all circumstances, including when you are deployed, TDY, or are otherwise not available due to military requirements. Because many military events are unscheduled and delays are unacceptable, it is imperative that all Soldiers ensure a plan is in place to provide proper and adequate care for their dependent(s). The U.S. Army provides a list of tasks to be accomplished IOT assist you in your preparations. See DA Form 5304 - Family Care Plan Counseling Checklist and AR 600-20, para. 5-3 for more detailed guidance.

Your FCP must contain the following forms (completed & current):

______ DD Form 2558 (DEC 2017) - Authorization to Start, Stop, or Change an Allotment

______ Letter of Instruction to the Guardian/Escort

______ DA Form 7666 (JUL 2020) - Parental Consent (IF APPLICABLE)

______ DA Form 7667 (JUL2020) - Family Care Plan Preliminary Screening (IF APPLICABLE)


You have 30 days from the date of this counseling, as an AD Soldier, to complete your FCP. If you do not understand what the forms require, get with me and we will figure it out. If you fail to complete your FCP within the designated time constraints IAW AR 600-20, para. 5-3, administrative actions may be initiated against you such as a bar to reenlistment or involuntary separation from military service IAW AR 635-200. You are required to keep your FCP current and update accordingly - such as a change of the designated care taker. You are required to conduct a yearly review of your FCP for accuracy and recertification during the anniversary of your birth month.


PLAN OF ACTION:

- SM understands the critical importance of readiness and the role of the FCP in ensuring his/her unit is ready to deploy and execute its mission within 24 hours, if necessary.
- SM will complete all necessary paperwork in a timely manner.
- Be prepared to deploy, be mission ready for TDY, and any other military event.
- Communicate with your NCO chain should any questions arise.
