PURPOSE OF COUNSELING
You are being counseled for failure to be at your place of duty (morning formation on 6 March 2014) and not using our chain of command.
This constitutes a formal counseling session IAW the requirements of AR 635-200, Active Duty Enlisted Administrative Separations, concerning the deficiency noted above. You are expected to correct this deficiency and to rehabilitate yourself into a productive soldier. Your behavior will be closely monitored and you will be given every opportunity to improve yourself.
KEY POINTS OF DISCUSSION
During your initial counseling you were told the dates and times you were required to be at formations. You signed that counseling acknowledging that you understood the directions. This is the second time you have failed to be present for formation. This failure to follow orders will not be tolerated.
You missed the 0700 morning formation. A soldier from the platoon was sent to your room to find you but there was no response to repeated knocking at the door. I also tried to call you on the phone but my call was not answered. At 0745 SFC XXXXXXX went to the barracks to see if you were in your room sleeping. He saw you running down the hall and when he asked you where you were going, you said the Emergency Room.
When I spoke to you later, you said that an NCO from another company drove you to the Emergency Roon and that you had to return to your room to get your ID card after the technician had taken your vital signs and told you no further care would be provided without a valid ID card. You also stated that you had called your squad leader, SGT XXXXXX, at 0630 to inform him that you were sick and going to the Emergency Room.
There is no record of a phone call from you to SGT XXXXXX that morning. When I asked for the name and phone number of the NCO who took you to the ER, you gave me his name but said that you didn't have to give me his phone number because it was stored in your privately owned phone. Even after I gave a direct order to give me his phone number so I could verify your story you refused.
As you know, there is nothing wrong with being sick or using the emergency room as long as the chain of command is informed. Even missing a formation can be excused if you have a good reason. But there is no excuse for lying. You have proven yourself to be untrustworthy and without integrity.

PLAN OF ACTION
PFC XXXXXX has been reminded of his responsibility to inform his chain of command of his whereabouts at all times. More counseling may follow if additional information concerning this incident surfaces.
PFC XXXXXX will make every effort to be at his appointed place of duty and is aware that failing to do so could result in separation from the Army IAW AR 635-200, Active Duty Enlisted Administrative Separations.
I will ensure that any order I give is lawful, is morally sound, and does not risk a Soldier's health. I will monitor PFC XXXXX's actions and assist with tips on ensuring he is always at his appointed place of duty.
I will provide and monitor corrective training if PFC XXXXX's performance continues to fail to meet standards.
I will recommend PFC XXXXX for UCMJ action and separation from the Army if corrective training fails to correct his performance.
